What you need to know

My client in the Public/Healthcare sector, based in Mullingar, Co. Westmeath is looking to recruit a Case Manager (Disability Services) to work in their facility.

This is a 6 month contract job initially

As the Case Manager, you will be responsible for providing appropriate service provisions for persons with disabilities and deal with day to day operational issues to ensure that the Service Agreement Contract in place follows policies/procedures.

Your New Job

Key duties & responsibilities:

  • Be responsible for the planning, coordination & delivery of services

  • Organise and coordinate supports and activities to optimise the functioning of people with multiple needs

  • Assist individuals to navigate available systems and services within their community

  • Ensure individuals, family members and external organisations are involved in the person centred planning process as appropriate

  • Review and evaluate client satisfaction with services

  • Develop links to the service development process

  • Provide reports in relation to complaints, FOI and other information requests

  • Participate in conferences, seminars and activities regionally & nationally to promote Case Management

  • Review existing services and develop new initiatives

  • Participate in Case Management training

  • Take part in research & development as appropriate.

What are we looking for?

  • 2-3 years experience working in a Case Management position or similar discipline

  • Have working knowledge and experience of HIQA standards

  • Knowledge of Service Level Agreements

  • Be aware of relevant legislative regulations and procedures

  • Support the development of services and implementations of policies
